The Soledad Community Health Care District (SCHCD) is a California Special District which owns and operates two stand-alone health care facilities in Soledad, California on a beautifully landscaped 3 ½ acre campus.

    • Eden Valley Care Center

      Our 59-bed Skilled Nursing Facility is designed and operated to give our residents the atmosphere of home.

    • “Rehab To Home” Program! Eden Valley Care Center is very pleased to announce that we have upgraded our rehabilitation services by teaming with Rob Snyder, Physical Therapist and owner of Rob Snyder Rehab Services. Rehab services available include physical therapy, speech therapy and occupational therapy. The “Rehab To Home” program goal is sending the patient home to their former activity and function.
    • Special Eden Valley features include a large, luxuriant garden, a 50’s style diner, a spacious atrium that brings the beauty of the outside world indoors, and a full service beauty shop. The Eden Valley menu is both elegant and nutritious. Rehabilitation Services and Patient Outcomes are some of the best in the region. Highly professional and genuinely caring therapists provide the highest levels of rehabilitation; which is widely recognized by Physicians and acute care Careplanners. Physical Therapy, Occupational Therapy, and Speech Therapy are provided in the most encouraging and supportive environments available.
  • Soledad Medical Clinic

    The Clinic is an outpatient Family Practice Rural Health Clinic. The Clinic’s medical professionals specialize in Family Practice, Women’s Health, and Babies and Children. The Clinic also provides digital X-ray, ultrasound and laboratory services. Medical care is provided by physicians, Certified Physician Assistants, Clinical Medical Assistants, and support staff. The clinic provided over 27,000 patient visits in 2010, and the practice continues to expand.

SCHCD is governed by a publicly elected, five member Board of Directors.  The District’s management team and day-to-day operations are directed by our Chief Executive Officer. The Soledad Community Health Care District Foundation, a 501(c)(3) nonprofit organization, enables SCHCD to receive tax deductible private donations and grants to supplement operations and meet community health care needs.
